Engine failure at 80ft after cylinder damage from running motor lean and dry

Pilot made altitude record attempt to 10,000 feet, ran engine lean and repeatedly dry (6-7 times). This damaged cylinder walls causing metal filings. Later experienced motor out at 80 feet on takeoff. Metal filings were shorting spark plug gap. Compression check showed 150 psi - no severe damage.

Pilot experienced a 40% right-side collapse at 15 feet altitude while flying along beach during a fly-in event. Despite correct brake input and wing recovery, insufficient altitude resulted in ground contact, damaging propeller and gas tank.

Spiral Recovery Gone Bad - Tree Collision

Pilot performing steep spiral for filming followed by quick recovery causing wingover. Rapid roll-out created slack lines and wing deformation. Fell through loose lines, wing reinflated but too low, impacted 20-foot tree at 6 feet height. Sustained ankle injury.

Boca Grande Island

Pilot decided to fly despite inland wind conditions at Boca Grande Island. Experienced turbulence during takeoff, possible engine/propeller issue during flight, lost altitude while trying to return to beach, and crash-landed from 10-15ft with no time to react, resulting in sprained ankle.

Hit vehicle in landing zone after takeoff with twisted riser

Pilot took off with a 360-degree twisted left riser without noticing during preflight. Despite having near-normal control, pilot lined up landing approach too close to their truck in a 10-acre landing zone and collided with it at low altitude, resulting in ankle pain and damage to the vehicle.

Pilot flying along shoreline with SE wind encountered turbulence near two 300ft tall towers. Wing started bouncing at 40ft altitude, entered parachutal stall at 20ft. Left corner of cage absorbed impact, pilot unharmed with minor cage damage.

Pilot experienced sudden engine failure at 50 feet AGL due to partial engine seizure caused by leaving primer bulb valve open and over-leaning high-speed carburetor screw. Safe landing made in rough rocky terrain.

Harness connector pin released at 75-100 feet AGL causing J-bar to rise and right brake toggle to be lost. Wing turned 90 degrees left. Pilot regained control and landed safely in crosswind conditions.

During forward launch with calm winds, the starter cord failed to fully retract and was ingested by the propeller, causing a loud bang and chunk of prop to break off along with damage to starter cup. Pilot shut down immediately and aborted takeoff. No injuries but hardware damage.

Experienced paramotor instructor conducting spiral maneuvers at approximately 1,000 ft experienced in-flight structural failure of paramotor lift arm attachments. Right-hand attachment failed first followed by left arm failure, causing uncontrollable left spiral. Pilot attempted recovery at low altitude but impacted ground with fatal injuries.

Fatal wing collapse during low-level aerobatic maneuver at UK Paramotor Championships, Tetbury

🇬🇧Chavenage Green Airstrip, near Tetbury, Gloucestershire, United Kingdom

Experienced paramotor pilot performing aerobatic maneuvers during free flying at UK National Championships. After slowing aircraft at low altitude, pilot rapidly applied speed bar while pulling on A-risers, inducing asymmetric collapse. Right wing collapsed, aircraft entered spiral dive with insufficient height for recovery, striking ground fatally.

Low altitude spin/stall after aggressive turn maneuver

Pilot entered unintentional spin and stall at approximately 50 feet altitude after applying heavy brake input following an upward exit from a right turn. Wing was low on energy when brake was applied. Pilot descended in stalled configuration and impacted ground with frame/cage absorbing impact.
